- Stomach and other conditions: Severe digestive problems, pancreatitis, gallbladder disease and dehydration-related kidney injury can occur. Use is not recommended with severe gastroparesis (very slow stomach emptying).
- Diabetes and heart rate: Discuss low-blood-sugar risk, especially with insulin or sulfonylureas. Report vision changes and a racing heartbeat at rest.
- Medicine review: Tell your provider about all medicines and supplements. Slower stomach emptying can affect oral medicines. Do not combine with another semaglutide product or GLP-1 medicine.

Important thyroid warning
FDA-approved semaglutide and tirzepatide products carry a boxed warning about thyroid C-cell tumors. Tumors occurred in rodent studies; whether these medicines cause them in people is unknown.
Do not use if you or a family member has had medullary thyroid carcinoma (MTC), or you have multiple endocrine neoplasia syndrome type 2 (MEN 2). Report a neck lump, persistent hoarseness or trouble swallowing. [2] [3]
Know when to get help
Call 911 for trouble breathing, facial or throat swelling, collapse, or other signs of a severe allergic reaction.
Stop the medicine and seek urgent care for severe, persistent abdominal pain, especially if it reaches your back, with or without vomiting.
Promptly contact a clinician for ongoing vomiting or diarrhea, inability to keep fluids down, very little urine, jaundice or vision changes. Do not wait for a portal reply in an emergency. [6] [3]

B12, glycine & your formulation
Somira lists B12 and glycine formulations for both medications. Your provider and pharmacist should confirm which ingredients, concentration and formulation are appropriate. Tell them about all allergies. [1]
Do not assume an added ingredient makes a compounded medicine equivalent to an approved medicine, improves weight loss or prevents side effects. FDA notes that the safety and effectiveness of combining semaglutide with additional ingredients, including B12, have not been established. [5]
Dosing, storage & safe use
- Confirm the dose before injecting. Ask your pharmacist to show the prescribed amount in milligrams, the corresponding volume, and the correct syringe marking. Do not copy “units” from a different vial or concentration.
- Follow your prescription. Do not increase the dose, inject more frequently, or restart after a treatment gap without instructions. Contact your care team for missed-dose guidance.
- Storage and delivery: Follow the pharmacy’s storage instructions. Do not use a compounded GLP-1 injection that arrives damaged or cloudy; contact the dispensing pharmacy.
- Opened multidose vials: FDA advises discarding within 28 days after first use, even if medicine remains or the compounder gives a longer period. Follow an earlier beyond-use date when applicable; clarify any conflicting directions with the pharmacy.
- Injection supplies: Use a new sterile needle and syringe each time and do not share supplies or medication.
