Prescription ED treatment
Sildenafil
A tablet used for erectile dysfunction (ED). It helps blood reach the penis during sexual arousal; it does not create an automatic erection. [1]

How it works & why it is prescribed
Sildenafil supports the blood-flow response to stimulation by blocking PDE5. It can help with erections but does not increase sexual desire, cure ED, or protect against pregnancy or sexually transmitted infections. [3]
What to expect & practical use
It is often used about an hour before sex; a high-fat meal can delay its effect. Follow your own prescription’s timing and dose rather than taking extra tablets when it seems slow to work. [3]
Tell your care team when treatment is ineffective or poorly tolerated. Do not add tadalafil or other ED medicines on your own. [2]

Safety & medicine interactions
Do not use with nitrates, riociguat, or a known allergy to sildenafil or its ingredients. ED medicines are unsuitable when your cardiac condition makes sexual activity unsafe. [2]
Alpha-blockers, blood-pressure drugs and medicines such as ritonavir or certain antifungals may require changes to treatment. Discuss kidney/liver disease, previous serious vision loss, or a history of prolonged erections. [2]
Prescription ED treatment
Tadalafil
An ED tablet that helps the blood-flow response to sexual stimulation. Effects may last up to 36 hours; that is an opportunity for a response, not a 36-hour erection. [1]

Daily and as-needed are different plans
Your prescriber chooses the schedule. Do not add an extra as-needed tablet to a daily regimen or combine it with another ED product. [4]
How it works & why it is prescribed
Tadalafil inhibits PDE5 to support erections during arousal. It does not increase desire or prevent pregnancy or sexually transmitted infections. Some approved tadalafil products also treat enlarged-prostate symptoms; that requires a separate clinical assessment. [5]
What to expect & practical use
Follow the schedule on your prescription. As-needed use is timed before sex; daily use is taken consistently without timing each dose to sex. Food does not meaningfully change how you take it. [5]
A longer action window does not remove interaction risks. Do not repeat a dose early or combine daily and as-needed regimens. [4]

Review alpha-blockers, blood-pressure medicines, certain antifungals and ritonavir with your prescriber. Kidney or liver problems may change suitability or dosing. Heavy drinking increases the risk of low blood pressure and dizziness. [4] [5]

Testosterone Cypionate
An injectable form of testosterone used to replace deficient hormone in appropriately diagnosed patients. It is not an ED tablet or a general treatment for ordinary aging. [6] [8]

Planning to have children?
Testosterone can reduce sperm production. Discuss fertility before starting. The Endocrine Society recommends against starting testosterone when fertility is planned in the near term. [9] [7]

How it works & who may benefit
Replacement therapy may address symptoms caused by confirmed testosterone deficiency. Diagnosis needs compatible symptoms and consistently low levels, usually confirmed with repeat morning testing. The cause of low testosterone also matters. [7]
It is not approved as an athletic-performance enhancer, and results are not guaranteed. [6]

Do not use with known product allergy, or serious heart, liver or kidney disease. Review clotting history, high hematocrit, untreated severe sleep apnea, urinary symptoms and recent cardiovascular events. [6] [7]
Blood thinners and diabetes treatment may need closer monitoring. Avoid self-directed hormone combinations or doses above the prescription. [6]
Human chorionic gonadotropin
HCG (10,000 IU)
HCG acts like luteinizing hormone, signaling the testes to make testosterone. Approved HCG labeling includes selected cases of hypogonadotropic hypogonadism—low hormone production from insufficient pituitary signaling. [10]

Fertility needs its own discussion
HCG is not interchangeable with testosterone. Do not assume adding it guarantees fertility or offsets testosterone’s sperm-suppressing effects. Discuss family-building plans with a clinician experienced in male reproductive health. [7] [9]

How it works & why it may be prescribed
HCG stimulates testosterone production rather than supplying testosterone directly. Your diagnosis determines whether that approach is appropriate; it is not a treatment for every cause of low testosterone. [10]
HCG is not FDA-approved for weight loss. It has not been shown to add weight-loss benefit to calorie restriction. [11]

Ask your prescriber and pharmacist to confirm the product, injection route, mixing instructions, storage and discard date. These vary by product; do not borrow instructions from another vial. [10]
