Compounded Medication Disclosure

Compounded medications are custom-prepared medications prescribed for individual patients when a licensed provider determines they are clinically appropriate.

Compounded medications:

• Are not approved by the U.S. Food and Drug Administration (FDA)
• Are not evaluated by the FDA for safety or effectiveness
• May differ from FDA-approved drugs in formulation, strength, stability, or availability
• Are prepared by state-licensed pharmacies or FDA-registered outsourcing facilities

Compounded medications are prescribed solely at the discretion of an independent, licensed healthcare provider.
